Join Interim HealthCare as a Hospice RN Case Manager – Flexible, ESOP in BATTLE CREEK, MI, US. Interim HealthCare of West Michigan is a locally owned and operated leader in post-acute care services in West Michigan. We are now hiring a Registered Nurse within our Hospice division.

Key Responsibilities
The RN Case Manager will conduct ongoing physical and psycho-social assessments of patients and families while developing collaborative plans of care. They are responsible for providing symptom management education, coordinating necessary supplies, and ensuring accurate documentation of services.
Requirements
Candidates must hold an active, unrestricted Registered Nurse license in the State of Michigan and possess at least one year of relevant RN experience. A valid driver's license, reliable transportation, and current CPR certification are also required.
