Outpatient Speech Language Pathologist, Home Health

Key Responsibilities

The Speech Language Pathologist will perform home visits to evaluate, diagnose, and provide therapeutic interventions for patients with speech, language, or swallowing difficulties. They are responsible for developing and revising treatment plans while educating patients and families to promote health and independent living.

Requirements

Candidates must hold a current Speech Language Pathologist license in Vermont and possess a certificate of clinical competence from the American Speech and Hearing Association. Prior experience with electronic medical records is preferred, and the role requires the ability to travel to patient homes within the designated territory.
