Role Overview
We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.
- What You’ll Do
- Routine & urgent assessments; chronic disease management
- Post-discharge follow-ups, med reconciliation, labs/diagnostics
- Coordinate with facility staff, specialists, families/POAs
- Same-day documentation; CMS/regulatory compliance
Schedule & Setting
Full-time position with a structured weekday schedule. Providers deliver on-site care across Stockton, Modesto, and Amador with strategically clustered routes, so you spend more time with patients and less time on the road.
Qualifications
- Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
- Geriatrics or facility-based experience preferred; EMR proficiency
Compensation
- Annual compensation range: $140,000–$175,000 (based on experience, schedule, and patient volume)
- Additional earning opportunities available based on coverage and panel needs
