Job Description
- Location: Grass Valley, CA
- Reports To: Medical Director
- Schedule: As needed for PTO, call-outs, and other provider coverage needs
- Work Arrangement: Onsite preferred; telehealth coverage may be considered when onsite coverage is unavailable
Job Summary
Under the direction of the Supervising Physician, the Primary Care Nurse Practitioner (NP) / Physician Assistant (PA) is responsible for delivering comprehensive medical care, including assessing, diagnosing, treating, and managing both acute and chronic health conditions.
This position will provide coverage when regularly scheduled providers are unavailable due to PTO, illness, or other absences. The ideal candidate will have the flexibility to provide onsite coverage at our Grass Valley office when needed. If onsite coverage is not available, qualified providers who are able to provide telehealth-only coverage may also be considered.
Essential Duties and Responsibilities
- Conduct thorough physical examinations and review patient medical histories.
- Order, interpret, and evaluate diagnostic testing.
- Diagnose and manage acute, chronic, and infectious conditions.
- Prescribe medications and appropriate treatment plans within scope of practice.
- Provide patient and family education regarding medical conditions, medications, treatment plans, and preventive health.
- Maintain accurate and timely documentation in the EMR.
- Collaborate with physicians, medical assistants, administrative staff, and other members of the care team.
- Promote preventive care and healthy lifestyle practices.
- Provide clinical coverage on an as-needed basis due to provider PTO, illness, or other absences.
- Provide onsite patient care when available; telehealth coverage may be utilized when appropriate.
Qualifications
- Active and unrestricted California Nurse Practitioner or Physician Assistant license.
- Board certification as an NP or PA.
- Clinical experience in primary care, urgent care, or family medicine.
- Strong clinical judgment and communication skills.
- Ability to work independently while collaborating with the clinical team.
- Flexible availability for PRN coverage.
- Ability to provide onsite coverage strongly preferred.
