Key Responsibilities

Serve as primary advocate for participants and caregivers, coordinating services to support social, emotional, and health-related needs.

Develop, implement, and update individualized social service care plans, Behavioral Plans progress notes, and discharge plans as required by regulations.

Coordinate services with other agencies, caregivers, and health providers to ensure continuity of care.

Conduct family and care planning meetings; participate in interdisciplinary and clinical meetings.

Requirements
