Conduct individual and group psychotherapy sessions for residents in skilled nursing facilities while developing evidence-based treatment plans. Collaborate with interdisciplinary teams and facility staff to monitor patient progress and maintain accurate clinical documentation.
Candidates must hold an active and unrestricted license as a Licensed Professional Counselor in the state of practice. Proficiency in electronic health records and the ability to provide in-person services at assigned facilities are required.
