Provide occupational therapy services to home health patients, including assessment, planning, and intervention. Coordinate care with physicians and educate patients and caregivers on treatment regimes.
Requires at least one year of OT experience and one year of recent home health experience. Must be proficient in HCHB charting and possess a valid driver's license and BLS certification.
