The Nurse Practitioner will provide home-based palliative symptom assessment, treatment, and ongoing goals of care discussions for patients with advanced illnesses. They will work within an interdisciplinary team to coordinate care, facilitate family meetings, and manage complex chronic disease states.
Candidates must hold a Master's degree and be a licensed Nurse Practitioner in New York State with current registration. A minimum of 3 years of experience in palliative care or geriatrics and national board certification are required.
