RN QRM Acute Transitional Case Manager - Multiple Part-Time and Full-Time Positions Available

Key Responsibilities

The Acute Transitional Case Manager coordinates care for complex patients to facilitate safe transitions from acute care settings to home or alternative facilities. They develop individualized care plans, perform telephonic assessments, and collaborate with healthcare teams to ensure continuity of care and avoid discharge delays.

Requirements

Candidates must have at least three years of acute hospital discharge planning or case management experience as an RN. A high school diploma or GED is required, along with a valid Registered Professional Nurse, Licensed Clinical Social Worker, or Licensed Master Social Worker license in Georgia.
