Key Responsibilities

• Position will be split between both locations, with a dedicated patient panel.

• Average of 17-20 patients per day; no nights or weekends (Extended clinic hours 7AM-5:30PM).

• 1.0 FTE = 36 Patient contact hours per week + administrative time.

• Shared call amongst the providers; Phone call only - triaged through MyNurse line.

Requirements

• MD or DO.

• Board Eligible or Board Certified in FM.

• Ability to obtain and hold an unrestricted license to practice in the state of Wisconsin.

• Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the physician’s field of practice.
