Join UHS as a SOCIAL WORKER (MSW) in Grand Forks, North Dakota. Responsibilities Social Worker (MSW)- Behavioral Health Horizon Health Corporation is seeking a Social Worker (MSW) for or our BRAND-NEW unit called Altru Behavioral Health Center in Grand Forks, ND. This unit will be a 24-bed inpatient unit for both adult and pediatric patients.
Key Responsibilities
- Social Worker (MSW)- Behavioral Health.
- Designs and implements a treatment plan of care for patients based on a thorough assessment of the patient’s symptom presentation, professional/referral input and all other available data.
- Participates in referral development processes and is an active member of the referral development team.
- Actively involved in assessing patient progress and communicating results and trends to supervisor and treatment team.
Requirements
- Master's Degree (Master of Social Work - MSW) required.
- Minimum five years' experience.
- Completes competent, clear progress notes and assessments according to program policies and procedures.
- Crisis prevention experience.
Work Schedule
- Full-Time position.
