Overview
THE BEHAVIORAL HEALTH COORDINATOR IS A KEY MEMBER OF THE BEHAVIORAL HEALTH TEAM RESPONSIBLE FOR MANAGING REFERRALS, SUPPORTING CONTINUITY OF CARE, AND HELPING PATIENTS NAVIGATE BEHAVIORAL HEALTH SERVICES.
THE ROLE PROVIDES NON-CLINICAL CARE COORDINATION, PATIENT FOLLOW-UP, AND OPERATIONAL SUPPORT WHILE FACILITATING ACCESS TO SERVICES.
GIVEN THE SENSITIVE NATURE OF BEHAVIORAL HEALTH INFORMATION, THIS POSITION IS ENTRUSTED WITH MAINTAINING A HIGH LEVEL OF CONFIDENTIALITY AND SAFEGUARDING PATIENT INFORMATION IN ACCORDANCE WITH HIPAA, ORGANIZATIONAL POLICIES, AND APPLICABLE PRIVACY REQUIREMENTS.
THE BEHAVIORAL HEALTH COORDINATOR REPORTS TO THE ASSOCIATE DIRECTOR OF CLINIC ADMINISTRATION OR DESIGNEE AND WORKS CLOSELY WITH THE DIRECTOR OF BEHAVIORAL HEALTH AND BEHAVIORAL HEALTH CLINICANS.
Essential Functions and Responsibilities
- Manage internal and external Behavioral Health referrals from receipt through completion, including reviewing referrals for required information, obtaining missing documentation, addressing insurance and authorization requirements, and coordinating access to appropriate Behavioral Health, psychiatric, substance use, community-based, or other support services.
- Maintain ongoing oversight of referral status and conduct timely follow-up with patients, referring providers, internal teams, and external organizations to address outstanding needs, confirm access to services, obtain follow-up documentation, and ensure closure of the referral loop.
- Conduct patient outreach and non-clinical care coordination as directed by the Director of Behavioral Health or designated clinical staff, including follow-up related to missed appointments, referrals, transitions in care, and other identified care needs.
- Assist patients with navigating barriers to care by coordinating next steps, helping resolve administrative or access-related issues, and connecting patients to appropriate organizational and community resources.
- Coordinate warm handoffs and follow-up between patients, Behavioral Health specialists, internal departments, and external service providers to promote continuity of care and reduce delays or gaps in services.
- Reinforce clinician-directed follow-up plans and administrative next steps while promptly escalating clinical questions, safety concerns, changes in patient condition, or other matters requiring assessment by an appropriate Behavioral Health clinician.
- Maintain accurate and timely documentation of referrals, outreach attempts, care coordination activities, barriers, resource connections, and follow-up outcomes within the Electronic Health Record (EHR) and designated tracking systems.
- Provide administrative support to Behavioral Health Clinicians and leadership, including working assigned patient lists, coordinating follow-up activities, preparing non-clinical correspondence or documents, maintaining departmental records, and assisting with special projects and other clerical needs.
- Facilitate patient access to Behavioral Health services, including scheduling appointments, maintaining accurate provider schedules, insurance eligibility verification, registration, intake documentation, and maintenance of accurate patient information in the EHR.
- Provide front-desk and reception support for Behavioral Health services as needed, ensuring patients receive professional, compassionate, and confidential assistance throughout their visit.
- Maintain strict confidentiality and exercise a high level of discretion when handling Behavioral Health information, ensuring protected health information is accessed, disclosed, transmitted, and discussed only as appropriate and in compliance with HIPAA, organizational policies, and applicable privacy requirements; promptly report or escalate suspected privacy or confidentiality concerns.
- Perform other duties as assigned in support of Behavioral Health and clinic operations.
Qualifications
- Bilingual Preferred: Spanish Speakers
- High school diploma or equivalent required; degree in psychology, social work, human services, healthcare administration, public health, or a related field preferred.
- Minimum two (2) years of experience in healthcare, behavioral health, patient access, referral coordination, care coordination, or a similar patient-facing administrative role; behavioral health experience preferred.
- Experience with patient registration, appointment scheduling, insurance eligibility, referrals, and Electronic Health Record (EHR/EMR) systems preferred.
- Strong organizational skills with excellent attention to detail, accuracy, follow-through, and the ability to manage multiple priorities in a fast-paced environment.
- Working knowledge of medical and behavioral health terminology, healthcare documentation, HIPAA privacy requirements, and appropriate handling of protected health information.
- Excellent customer service and communication skills, with the ability to interact professionally and compassionately with patients, providers, community partners, and other healthcare professionals.
- Ability to work effectively as part of an interdisciplinary healthcare team and exercise appropriate judgment when identifying and escalating patient needs or concerns.
- Ability to maintain professionalism, discretion, appropriate boundaries, and composure when supporting patients who may be experiencing behavioral health concerns, distress, or barriers to care.

- Must take yearly flu shots or wear flu mask during flu season for patient-facing positions and test for tuberculosis as required by the Centers for Disease Control and Prevention.
