Registered Dietitian - This position will work at the Vadnais Heights, Roseville, Blaine, North Branch and Maple Grove branches. Position Overview The Registered Dietitian is an experienced clinician who serves as a member of the clinical care team, meeting the needs of each patient based on their unique Plan of Care.

In this role the Registered Dietitian plans, organizes and conducts dietetic education and supports patients and their caregivers as a member of the interdisciplinary team with expertise in nutritional care.

Essential Functions and Skills Develops and implements a nutritional plan of care which is based on a comprehensive assessment of nutritional needs as part of the patient's total plan of care. Evaluates and reports results appropriately.

The dietitian works with the referring physician and interdisciplinary group to provide appropriate counseling to individuals and families/caregivers in the home setting.

They evaluate and instruct on nutritional principles, dietary plans, food selection and economics, and adapting plans to the individual's illness, as indicated. They will reevaluate nutritional care by providing home follow-up care either at the patient's residence or by phone.

Communicates appropriate dietary history and nutritional care data through written record systems. Records subjective and objective information, assessment, treatment plan, and current progress notes in the patient's clinical records including a discharge summary. Attend interdisciplinary group meetings.

Comply with the organization's policies and procedures. Maintains confidentiality and safety of patient records so that no information is divulged to other than proper users. Actively participates in quality assessment performance improvement teams and activities. Performs other duties as assigned.

Qualifications

Requirements/Qualifications Minnesota Registered Dietitian Credential. Master’s degree in dietitian studies from an accredited institution. Ability to pass background, consumer report, and drug test. Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required. Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting. The hourly base range for this role is $32-$37 per hour.

An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets.
