We are currently recruiting for a compassionate Home Health Physical Therapist (PT) to reliably serve patients throughout the Lower Bucks County, PA area. This is a high-priority area, and we’re offering enhanced pay rates plus a quarterly retention bonus for Physical Therapists interested in supporting clients in these communities.
What You’ll Do
As a Jefferson Health at Home by BAYADA Physical Therapist, you’ll use your clinical skills and compassionate heart to treat patients where they want to be the most – in the comfort of their own home. As a Jefferson Health at Home by BAYADA PT you will:
Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
What You Should Have
- Active and current license to practice as a professional Physical Therapist (PT) in the state of PA.
- Previous home healthcare experience welcomed, but not required.
Ability to Travel to Cases as Assigned
A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.
The ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability.

Additional Information
- Base Pay, depending upon qualifications, status and subject to negotiation:
- $65.00 - $70.00 per point (101,400 - $109,200 annual earning potential)
Territory Differential for Lower Bucks County - $70.00 - $73.00 per point ($109,200 - $113,880 annual earning potential) plus quarterly retention bonus payout. Must meet productivity goals to receive retention bonus.

Key Responsibilities
The Physical Therapist will perform home visits to conduct diagnostic tests, develop individualized care plans, and implement therapeutic interventions. They are also responsible for educating clients and families while accurately documenting all care provided using a tablet.
Requirements
Candidates must hold an active and current license to practice as a Physical Therapist in the state of Pennsylvania. The role requires the ability to travel to assigned cases and a compassionate, professional approach to patient care.
