Job Details: Job Location: Main Hospital Campus Headquarters - Madison, WV, Position Type: Per Diem, Salary Range: Undisclosed, Pharmacist - Per Diem Boone Memorial Health is seeking a dedicated and knowledgeable Pharmacist to join our Pharmacy team.
The Pharmacist will be responsible for safely and accurately dispensing medications while providing medication information and guidance to patients and healthcare professionals.
Position Summary The Pharmacist reviews prescriptions, prepares and dispenses medications, monitors medication therapy, maintains accurate pharmacy records, and collaborates with physicians and other healthcare professionals to promote safe and effective patient care.

Responsibilities
Review prescriptions for accuracy, appropriateness, drug interactions, dosage, and potential side effects. Compound and dispense medications according to physician orders and applicable regulations.
Provide medication counseling and education to patients regarding proper use, dosage, storage, and potential side effects. Maintain accurate patient profiles, pharmacy records, inventory records, and controlled substance documentation.
Ensure medications are properly prepared, packaged, labeled, stored, and disposed of according to hospital policy and legal requirements. Collaborate with physicians, nurses, and other healthcare professionals regarding medication selection and treatment plans.
Monitor medication use and prescribing trends to promote patient safety and appropriate medication utilization. Order pharmaceutical and medical supplies and maintain appropriate inventory levels. Provide patient education and specialized medication services as appropriate.
Assist with the training and supervision of pharmacy interns and other pharmacy personnel. Perform administrative and other pharmacy duties as assigned.
Qualifications Degree in Pharmacy from an accredited School of Pharmacy required. Current West Virginia State Board of Pharmacy Registered Pharmacist License required. Prior hospital pharmacy experience preferred. Strong knowledge of medications, drug interactions, pharmacology, chemistry, and medical terminology.

Requirements
This position requires the ability to stand or sit for extended periods, walk, climb stairs, reach, bend, twist, push, pull, squat, grasp, and perform repetitive arm and hand movements. The employee must be able to lift up to 15 pounds using proper lifting techniques. Travel Up to 15% travel may be required within West Virginia and surrounding areas.
