Key Responsibilities
The Hospice RN Case Manager performs physical and psychosocial assessments on patients and develops individualized plans of care. They collaborate with physicians and multidisciplinary teams to manage pain and provide emotional support to patients and their families.
Requirements
Candidates must be a licensed Registered Nurse in the State of Indiana with at least two years of RN experience. One year of hospice experience is preferred, and the role requires a valid driver's license with reliable transportation.
