Job Description
To support, protect, and empower vulnerable adults or adults at risk of homelessness by assessing their needs, coordinating general welfare benefits, state/federal services, and a care plan while ensuring safeguarding procedures are followed.
The role involves working collaboratively with individuals, families, healthcare providers, and partner agencies to promote independence, wellbeing, and safety.
Coordination with all tribal community services departments inclusive of housing, education, and general resources to provide wrap around services for at risk or vulnerable adults.
Essential Duties
- Conduct comprehensive assessments of vulnerable adults’ physical, emotional, social, and environmental needs.
- Conduct compliance reviews of general welfare benefit misuse and develop a holistic approach to respond to misuse in compliance with the benefit Misuse Policy.
- Develop, implement, and review person-centered care and support plans.
- Identify safeguarding concerns and respond appropriately in accordance with legislation, policy, and procedures.
- Investigate allegations of abuse, neglect, exploitation, or self-neglect involving vulnerable adults.
- Coordinate multi-agency interventions with healthcare professionals, housing providers, law enforcement, advocacy services, and community organizations.
- Maintain accurate, timely, and confidential case records and reports.
- Support service users in accessing benefits, housing, healthcare, mental health services, and community resources.
- Advocate for the rights, dignity, choice, and independence of vulnerable adults.
- Participate in case conferences, safeguarding meetings, and court proceedings where required.
- Monitor and review outcomes to ensure effective support and risk management.
- Maintain professional standards in line with relevant social work regulations and ethical guidelines.
Evaluate the effectiveness of the care plans and adjust as needed. Develop and implement individual care plans in partnership with elders and their families.
- Connect adults with essential community resources with face-to-face hands-on assistance with in the field.
- Work closely with groups and individuals to help build and maintain positive relationships.
- Address concerns of abuse, neglect, or exploitation concerns and work with appropriate authorities when necessary.
- Provide culturally sensitive crisis intervention, grief support, and guidance
- Provide advocacy and support with applications, referrals, and service coordination.
- Collaborate with medical professionals and other service providers.
- Make appropriate recommendations and assist the supervisor with planning of department goals and objectives.
Maintain client records and documentation. Gather, compile, and analyze data and prepare reports.
Additional Duties
Participate in interdepartmental meetings and training as appropriate. Participate in continuing education as required and/or needed to fulfill job duties.

Education & Experience
Education Required
Bachelor’s degree in social work, human services, gerontology, or a related field
Experience
Experience with case management software.
Minimum of 2 years of experience working with homeless or at risk populations, preferably in a tribal or Indigenous community setting.
Certifications/Licenses
- Must possess and maintain a valid and unrestricted Washington State driver’s license.
- Current social worker licensure as required by WA state.
Other Requirements
Upon hire, successfully complete and pass required training, such as defensive driving, mandatory reporting, and HIPAA compliance.
Current CPR/1st Aid card, TB immunization, and blood borne pathogens training preferred; required to obtain all within first six months of hire.
Must be able to pass a thorough pre-employment background investigation.
Must agree to be subject to the Tribe's Controlled Substance and Alcohol Testing Policy to include pre-employment screening
Work Environment
The work is performed in a variety of settings including, but not limited to, client homes, adult family homes, and the usual office environment. Employees may be required to travel to clients' place of residence, courts, or various visitation and/or other locations of applicable meetings or events.
Travel on behalf of the Tribe is a job requirement. Travel for case management and trainings may require local and/or out-of-state travel along with overnight stays.
Regular interaction with high-risk clients including home visits that may be unaccompanied.
Tribal Preference
The Snoqualmie Indian Tribe follows the Tribal Hiring Preference as outlined in tribal policies, including the Tribal Employment Rights Ordinance (TERO). TERO sets forth certain hiring preferences for qualified individuals who are enrolled in a federally recognized Indian tribe, as well as, in some cases, preference for certain family members of those individuals.
Additional Information
Salary, compensation will be determined based on experience and starts at $76,159.67. This position is a regular full-time position and employment is at will.
