- Medical Social Worker- MSW Schedule/Status: Full-Time, Exempt (Salary) Department: Hospice/Home Health Service Area: Lewiston, Maine Role: Medical Social Worker - MSW Work Setting: Community Care Facilities and Homes What You’ll Do: Assesses and identify the client’s holistic health care needs/concerns as part of a family and community. Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care. Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols. Implement patient care plan activities based on treatment goals towards independent living for the patient/family. Serve as resource for staff education & training, participate in case conferences as needed. Per Diem LCSW to support Palliative Care, Hospice, and Home Health What You’ll Bring: Master’s degree from a school of social work accredited by the council on social work education. Maine Board of Social Work Licensure- MSW or LCSW required Current CPR card required. One or more years of experience as a master’s level social worker in a health care setting. A valid Drivers License and reliable car with insurance.

Key Responsibilities
The Medical Social Worker assesses the holistic health and emotional needs of patients and their families to develop and implement effective care plans. They collaborate with the interdisciplinary health care team to ensure quality outcomes and provide education and training to staff.
Requirements
Candidates must hold a Master’s degree in Social Work and possess a valid Maine Board of Social Work Licensure (MSW or LCSW). At least one year of experience in a health care setting and a valid driver's license with reliable transportation are required.
