Key Responsibilities

The Social Worker will coordinate individualized psychosocial care plans and serve as the primary contact for resident advocacy and regulatory compliance. They will also manage transitions of care, facilitate care plan meetings, and provide backup support for admissions and administrative tasks.

Requirements
