Key Responsibilities

The RN Case Manager is responsible for creating, implementing, and evaluating care plans while collaborating with interdisciplinary teams to optimize patient outcomes. They manage readmissions, length of stay, and ED utilization while advocating for patients and their families throughout the care continuum.

Requirements

Candidates must hold a Bachelor’s Degree in Nursing or a combination of a Bachelor’s degree and a Nursing Diploma, with at least 3 years of clinical nursing experience. A current Pennsylvania RN license or multi-state licensure through the eNLC is required.
