Working in close coordination with physicians, psychiatrists, and other behavioral health providers, you'll help guide treatment decisions, support care transitions when needed, and connect patients with additional resources.

Your work will directly improve access to mental health care within primary care settings and strengthen outcomes across both physical and behavioral health.
