Join a physician-led organization where the focus is on patient care, not administrative complexity. At Millennium Physician Group, we've created an environment that empowers providers to practice at the top of their license, supported by a collaborative care model and industry-leading resources that allow you to spend more time with patients and less time navigating bureaucracy.

As one of the nation's leading independent physician groups and a recognized leader in value-based care, Millennium Physician Group offers the stability of an established network combined with the support and autonomy providers value.

As a Primary Care Advanced Practice Provider, you'll work alongside physicians and interdisciplinary care teams to deliver comprehensive, patient-centered care while helping drive quality outcomes across a growing patient population.

This is an outstanding opportunity for an APRN or PA seeking clinical autonomy, meaningful patient relationships, and the ability to make a lasting impact on patients' health and well-being.

How Will You Make an Impact & Requirements‎

At Millennium Physician Group, we're transforming primary care through an innovative, patient-centered value-based care model.

As a Primary Care Advanced Practice Provider, you will partner with physicians, care managers, specialists, and support teams to provide comprehensive care for patients with acute, chronic, and palliative care needs.

Supported by advanced technology, coordinated care resources, and a robust clinical infrastructure, you'll have the opportunity to focus on delivering exceptional care while improving quality outcomes and enhancing the patient experience.

This role is ideal for a provider who is passionate about preventive medicine, chronic disease management, population health, and coordinated care. Through proactive patient management and strong care coordination, you'll help improve outcomes, close care gaps, and reduce unnecessary emergency department visits and hospitalizations.

What You'll Do

Provide comprehensive primary care services for assigned patients in collaboration with physicians and interdisciplinary care team members.

  • Perform patient assessments, physical examinations, and comprehensive care planning.
  • Evaluate, diagnose, and treat acute and chronic medical conditions using evidence-based clinical guidelines.
  • Develop risk management plans and provide chronic care management and palliative care services within scope of practice.
  • Interpret laboratory studies, imaging results, and diagnostic findings to guide treatment decisions.

Prescribe medications and order home health services, therapies, and durable medical equipment (DME) as appropriate.

Identify and address quality measures, care gaps, and HCC documentation opportunities to support value-based care initiatives.

Provide interventions that help prevent unnecessary emergency department visits and hospital admissions.

Educate patients and caregivers on disease management, medication adherence, symptom management, preventive care, and advanced care planning.

Coordinate care across primary care providers, specialists, caregivers, home health agencies, skilled nursing facilities, and community resources.

Identify patients who would benefit from skilled nursing, therapy services, behavioral health support, social services, or other community-based resources.

Ensure accurate, timely, and compliant documentation and billing within the electronic medical record.

Collaborate with assigned Medical Assistants and practice leadership to support workflow efficiency, staff development, and quality improvement initiatives.

Consult with supervising physicians regarding complex diagnoses, treatment plans, and patient management decisions as needed.

Qualifications

  • Active Florida Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) license in good standing.
  • National Board Certification as an APRN or PA.
  • Current DEA registration or eligibility to obtain.
  • Experience in Primary Care, Family Medicine, Internal Medicine, Geriatrics, or a related clinical specialty preferred.
  • Strong clinical assessment, diagnostic reasoning, and chronic disease management skills.
  • Knowledge of value-based care, population health management, quality measures, and HCC documentation.
  • Experience working with electronic health records and clinical technology platforms.
  • Excellent patient communication, education, and care coordination skills.
  • Commitment to evidence-based, patient-centered care.
  • Strong clinical judgment and ability to work collaboratively within a multidisciplinary environment.
  • Experience with AthenaOne EMR is a plus.
