Public Service Loan Forgiveness (PSLF) Eligible

This position qualifies for federal student loan forgiveness. After 10 years of qualifying payments while working full-time for a nonprofit employer, the remaining balance on your Direct Loans may be forgiven.

Key Responsibilities

  • Perform initial and ongoing psychosocial/bereavement evaluations of patients/family/caregiver and assist in the development/implementation of the plan of care.
  • Is a core member of the interdisciplinary group (IDG) and actively participates in IDG meetings for development and evaluation of the patient’s plan of care.
  • Implement a plan of care for each assigned patient/family/caregiver unit and evaluation of that plan of care.
  • Assist patient/family/caregiver in determining of goals of care including completion of Advance Directives and POLST forms as needed.

  • Equipment: General office equipment Travel: Requires travel outside of the workplace at least weekly; the employee’s personal vehicle may be used.
  • Driving infractions will be monitored in accordance with organizational policy.
