The Nurse Practitioner will focus on quality resident care, acting as an educator and clinician to develop the multi-disciplinary Village team and drive clinical quality improvement outcomes. Responsibilities include supporting inter-disciplinary teams, guiding assessments, interpreting diagnostics, leading resident-centered palliative care, and participating in quality initiative reviews.
Candidates must hold current registration as a Nurse Practitioner with the College of Nurses of Ontario and possess experience completing comprehensive geriatric assessments and leading/mentoring care professionals. Required certifications include current Heart Save Level of CPR and Current AIS Certification.
