Key Responsibilities
The Utilization Case Manager conducts in-home health assessments and coordinates care for members to identify unmet needs and treatment barriers. They also provide patient education, track findings, and collaborate with Medical Directors to ensure effective service delivery.
Requirements
Candidates must hold a current, unrestricted RN license in Wisconsin and possess at least two years of experience in acute care or home health case management. Proficiency in Microsoft Office and electronic medical records is required, along with reliable transportation for local travel.
