Baywood Home Care is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate care, support patients and families, and help individuals maintain their independence in the comfort of their own homes. Flexible Scheduling: Create a part time schedule that works for you, up to 20 hours/week. Hours are flexible, and shifts can be coordinated around both your availability and patient needs!

Optional Paid On-Call Opportunities: On-call participation is not required, but additional compensation is available for nurses who choose to take on-call shifts

What You'll Do

• Conduct patient assessments and develop individualized care plans.
• Coordinate services with physicians, therapists, caregivers, and community resources.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and families on medications, treatment plans, and wellness strategies
• Maintain accurate clinical documentation and regulatory compliance.

Qualifications

• Current Minnesota Registered Nurse (RN) license, required.
• Home health, case management, or transitional care experience, required.
• Strong assessment, communication, and care coordination skills.
• Must be 21 years of age or older.
