Key Responsibilities

Provide advanced palliative care to patients in their homes by performing in-depth assessments and developing individualized treatment plans. Collaborate with the Palliative Medical Director and interdisciplinary team to manage complex medical needs and guide families through treatment stages.

Requirements

Requires a Master of Science in Nursing and at least two years of Nurse Practitioner experience. Must hold a current RN license, Nurse Practitioner certification, and a valid driver's license with automobile insurance.
