This opportunity offers a 100% outpatient setting, strong clinical support, and the ability to build a patient panel within a mission-driven, community-focused health system. --- Practice Highlights

• Provide comprehensive primary care for patients across the lifespan
• Diagnose and manage acute and chronic conditions
• Emphasize preventive care and population health management
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Maintain timely and accurate documentation
