The Social Worker will conduct psychosocial assessments, provide counseling to residents and families, and support the admissions and transition process. They will also collaborate with the interdisciplinary team on care planning, documentation, and discharge coordination.
Candidates must hold a Bachelor's degree in Social Work or a related field and possess a current CSW certificate. At least one year of interdisciplinary team experience, preferably in long-term care or hospice, is required.
