Key Responsibilities

The Physical Therapist will plan and administer personalized rehabilitation programs for patients in their homes to improve strength and mobility. They are also responsible for documenting patient progress and coordinating care with physicians and the multidisciplinary team.

Requirements

Candidates must hold a current and unrestricted Physical Therapy license and CPR certification. Experience collaborating with a team of healthcare professionals is required, with at least one year of experience preferred.
