Key Responsibilities
Provide primary and consultative care to geriatric and medically complex patients in their homes or care settings. Conduct comprehensive medical evaluations, manage treatment plans, and coordinate with interdisciplinary teams to improve patient outcomes.
Requirements
Requires a master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant with a current state license. Candidates must have reliable transportation and a valid driver’s license for daily travel within the service area.
