Social Worker - PRN - Rocky Hill, CT

Key Responsibilities

The Social Worker is responsible for assessing the psychosocial status of patients and families related to terminal illness, communicating findings, and carrying out social evaluations including family dynamics and risk assessments. They will also counsel patients and families, provide crisis intervention, educate on cultural needs, and assist with advanced directives and practical/environmental needs.

Requirements

Candidates must be a graduate of a master’s or bachelor’s program in social work accredited by the Council on Social Work Education and possess the required state licensure, education, training, and qualifications. Previous home care/hospice experience is required, hospice care experience is preferred, along with good communication skills and a valid driver's license with insurance.
