Key Responsibilities

The Primary Care Physician provides equitable value-based healthcare to Medicare patient populations at neighborhood primary care centers. Responsibilities include diagnosing patients, administering pharmaceutical treatments, and collaborating with leadership to meet health quality goals.

Licensure & Certifications

Candidates must be an M.D. or D.O. with board certification or eligibility in Internal or Family Medicine and hold an active state license and DEA license. Fellowship training in Geriatrics and experience in value-based healthcare models are preferred.
