Transitions At Home is a trusted provider of home health services, committed to delivering high-quality, compassionate care to patients in the comfort of their homes we are seeking an experienced RN Case Manager to support our care team and play a vital role in coordinating and delivering patient-centered care..

What does a Transitions at Home RN Case Manager do?
Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Hours: Full-Time
* Starting at 30 hours per week 8:00 am - 4:30pm
* Monday through Friday
* No Nights or Weekends
* Limited on-call rotation

Qualifications / Job Requirements
* Active Wisconsin Registered Nurse License
* Two (2) years’ nursing experience, 1 year in Home Health (preferred)
* CPR Certification
* A valid Wisconsin Driver's License and Car Insurance

Candidates must hold an active Wisconsin Registered Nurse license and possess at least two years of nursing experience, with one year in home health preferred. A valid Wisconsin driver's license, car insurance, and CPR certification are also required.
