Job Description Summary ‎ Responsible for managing patient care and treatment through in-home clinical visits, in close collaboration with the physician and interdisciplinary care team.

Patients trust us to deliver the right care at the right time, helping them recover safely at home and achieve better long-term health outcomes and quality of life. ‎ How will you make an impact & Requirements ‎ Field Nurse Practitioner – Home Care (San Fernando Valley & Antelope Valley) Full-Time | Monday–Friday | Field-Based We are seeking a compassionate Adult/Geriatric Nurse Practitioner to join our growing home-based care team in the San Fernando Valley & Antelope Valley area.

This is a meaningful field role focused on caring for patients where they are most comfortable — at home — during some of the most important moments in their recovery.

You will manage an assigned panel of patients receiving acute and transitional care in the home, including individuals recently discharged from the hospital, emergency department, or skilled nursing facilities, as well as medically complex, homebound patients with lower acuity needs.

What You’ll Do Conduct in-home health assessments and focused physical exams Manage acute and post-acute conditions for patients transitioning from hospital, ER, or nursing facilities Develop, implement, and document individualized care plans Order, interpret, and evaluate diagnostic tests Prescribe medications and treatments within scope of practice Collaborate closely with physicians, care managers, and interdisciplinary team members Educate patients and families to support recovery, safety, and long-term health at home Maintain timely, accurate clinical documentation Service Area This position serves patients throughout the San Fernando Valley & Antelope Valley including: San Fernando Valley (SFV) Northridge Van Nuys Woodland Hills Reseda Panorama City Antelope Valley (northern LA County) Palmdale Lancaster Schedule & Compensation Schedule: Full-time, Monday–Friday Salary Range: $119,312.60 – $178,967.80 annually Bonus eligible based on individual and company performance

Qualifications

Master’s degree in Nursing (MSN) Adult/Geriatric Nurse Practitioner certification Minimum 1 year of Nurse Practitioner clinical experience (home-based or transitional care experience a plus) Active, unrestricted RN and NP license in the applicable state Current TB test or willingness to obtain prior to start ‎ Compensation: $119,313.00 to $178,968.00 Beware of fraudulent job postings: While Mosaic Health job advertisements may be found on many sites, our current openings page and its associated Workday account are the only places we accept applications for open roles.
