Key Responsibilities
The RN Case Manager conducts initial patient assessments, develops and revises individualized care plans, and provides ongoing nursing care and health education. They also coordinate with families and physicians to ensure effective symptom management and discharge planning.
Requirements
Candidates must hold a current and valid state RN license and possess at least one year of recent acute care experience. A valid driver's license, auto insurance, and BLS certification are also required.
