Key Responsibilities
This role coordinates and facilitates quality, cost-effective care for ESRD and CKD patients by acting as a liaison between patients, families, and healthcare providers. Requirements
Candidates must hold an A.D.N. degree or higher and a current Registered Nurse license, with at least five years of clinical nursing experience. Proficiency in case management, basic computer skills, and a valid driver's license are required for this field-based position.
