Key Responsibilities
The RN Case Manager leads the hospice care team to develop and manage patient Plans of Care while providing direct day-to-day patient care. They are responsible for administering treatments, maintaining clinical records, and coordinating with interdisciplinary teams to ensure patient comfort and quality of life.
Requirements
Candidates must hold a current Registered Nurse (RN) license and have the ability to pass background and drug screenings. A valid driver's license and access to a reliable vehicle are required, with preference given to those with at least one year of hospice or home care experience.
