Job description
Join The Elizabeth Hospice as a Hospice RN Case Manager – M-F, No Nights in Carlsbad, CA, US. The RN Case Manager is primarily responsible for providing professional nursing care to assigned patients, which includes adults, pediatric and perinatal. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care.

Key Responsibilities
The RN Case Manager provides professional nursing care to patients and develops, implements, and evaluates individualized plans of care in collaboration with an interdisciplinary team. They are responsible for supervising LVN and HA staff, performing clinical assessments, and ensuring timely documentation in the electronic charting system.
Requirements
Candidates must hold an Associate degree or higher in nursing and possess a current California RN license. Professional experience in hospice, oncology, or medical-surgical nursing is preferred, along with a valid driver's license and reliable transportation for field visits.
