The RN Case Manager conducts initial patient assessments, develops and revises individualized care plans, and coordinates with families to meet patient needs. They are responsible for administering prescribed treatments, providing health education, and managing discharge planning processes.
Candidates must hold a current and valid state RN license and possess at least one year of recent acute care experience in an institutional setting. A valid driver's license, auto insurance, and BLS/CPR certification are required, while a Bachelor's degree in Nursing is preferred.
