Assess and respond to suicide, violence, and safety risk following program protocols.

Bridge Program

Continue 1 to 2 recommended sessions at release and update the plan of care for community needs.

Carry out the phased community plan: Phase 1 (4 months), Phase 2 (3 months, weekly), and Phase 3 (2 months, 1 to 2 sessions monthly).

Coordinate with the care coordinator on transportation, referrals, and linkage to housing, employment, benefits, and recovery supports.

Complete phase-out planning with exit recommendations, additional resources, continuing therapy options, and mentorship programs.

Collaboration and Administration

  • Work with attorneys, social workers, and other healthcare professionals who refer clients.
  • Complete timely, compliant documentation in the electronic record.
  • Participate in case review, supervision, and quality and outcomes reporting.
  • Follow facility security, clearance, and confidentiality requirements.

Additional for LCSW

  • Serve as a clinical supervisor for ACSW and other associate clinicians, if qualified and assigned.
  • Handle more complex cases and assist with admission reviews.
