The physical therapist will perform clinical assessments, develop and revise plans of care, and provide direct treatment for geriatric patients in their homes. They are also responsible for documenting clinical notes, supervising therapy assistants, and educating patients and families on safety and equipment.
Candidates must hold a current Physical Therapist license and possess strong clinical and assessment skills. At least one year of home health experience is preferred, along with the ability to work independently and manage a fast-paced schedule.
