Careage Home Health is seeking a Physical Therapist to join our team!! Summary of Responsibilities

Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services.

  • Work in conjunction with all other home care personnel in patient care planning.
  • Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
  • Take initial patient history and make initial evaluation.
  • Consult with physicians regarding change in treatment.
  • Instruct patients and family in home programs and activities of daily living.
  • Participates in Quality Assessment and Performance Improvement activities as assigned.
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
  • Appropriately utilizes ICD-10 codes.

Requirements

  • Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
  • Currently licensed as a physical therapist in the state of Washington.
  • Two (2) years' experience as a physical therapist, preferred.
  • Home Health experience, preferred.
  • Detail oriented, self motivated and capable of efficient time management.
  • Critical thinker capable of working collaboratively with a team.
  • Valid WA State driver's license.

Key Responsibilities

The Physical Therapist will provide one-on-one patient care, develop plans of care, and conduct initial evaluations in a home health setting. They are also responsible for supervising physical therapy assistants and home health aides while ensuring accurate documentation and compliance with physician orders.
