Purpose of this position: The BHC is responsible for supporting and coordinating the behavioral health care of patients on an assigned patient caseload in partnership with the patient’s medical provider and a consulting psychiatrist (PCP).
Effective BHCs support the PCP in their role by coordinating the overall treatment plan, providing brief psychotherapy interventions, supporting medication management, alerting the PCP when the patient is not improving, and communicating with the psychiatric consultant regarding treatment changes.
This role will also systematically track treatment response and monitor patients (in person and/ or virtually) for changes in clinical symptoms and treatment side effects or complications.
