Clinical Care Social Worker

Key Responsibilities

The Clinical Care Social Worker coordinates patient care across the continuum by performing psychosocial assessments, crisis interventions, and discharge planning. They work collaboratively with multidisciplinary teams to ensure effective treatment outcomes and facilitate referrals to appropriate community resources.

Requirements

Candidates must hold a Master's degree in Social Work and possess current state licensure as an LGSW, LCSW, or LICSW in West Virginia, or LMSW/LCSW-C in Maryland. One to three years of relevant experience is preferred.
