Senior Living Physical Therapist

Key Responsibilities

The Physical Therapist will perform outpatient home health visits to provide rehabilitative care and develop individualized care plans for geriatric clients. They are responsible for assessing patient mobility, implementing therapeutic interventions, and documenting care using electronic medical records.

Requirements

Candidates must hold a current New Hampshire Physical Therapist license and have at least one year of recent clinical experience. Graduation from an accredited physical therapy program is required, along with strong organizational and interpersonal skills.
