Job Description

Harbor is seeking an Adult Psychiatric Nurse Practitioner to join our Toledo team! This position is responsible for providing comprehensive psychiatric assessment, diagnosis, treatment, and medication management for adult patients.

The Adult Psychiatric Nurse Practitioner delivers evidence-based, patient-centered care that supports positive clinical outcomes while promoting wellness, recovery, and overall quality of life.

This is a full-time position, working 40 hours per week.

Requirements

Education

  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited nursing program.
  • Completion of an accredited Nurse Practitioner program in a relevant specialty area.
  • Equivalent combination of advanced education, clinical training, and experience may be considered where applicable.

Experience

  • Must be proficient and accurate in computer use.

Licensure and Certifications

  • Current, active, and unrestricted CNP or CNS licensure in the state of Ohio.
  • Active DEA license.
  • Must maintain all licenses and certifications in good standing throughout employment.
  • BLS and NCI/CPI Certifications required within 90 days of employment.
  • Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier if applicable.

Preferred Qualifications

  • Two years of relevant clinical experience as a nurse practitioner or registered nurse.
  • Experience in patient assessment, diagnosis, treatment planning, and care coordination.
  • Prior experience working in interdisciplinary healthcare teams and fast-paced clinical settings.

Responsibilities

Essential Functions and Primary Responsibilities

1. Conduct patient assessments, diagnose conditions, and provide evidence-based treatment plans.

2. Prescribe medications and therapies within scope of practice and sends prescriptions in a timely manner per protocol.

3. Educate patients and families on disease management, preventive care, and healthy lifestyle choices.

4. 4. Collaborate with physicians, nurses, and interdisciplinary teams to ensure coordinated, high-quality care.

5. Participate in quality improvement initiatives and maintain accurate and timely clinical documentation, including psychiatric assessments, progress notes, treatment plans, and medication orders.

6. Ensures appropriate client hospitalizations (voluntary and involuntary) when indicated, including Harbor clients as required.

7. Responds appropriately to emergency situations.

8. Utilizes EHR systems and related technologies, including e-prescribing, voice recognition, and self-documentation tools.

9. Meets or exceeds productivity expectations for billable services, signs and co-signs billing documents and treatment plans as required.

10. Actively participates in Medical Services Team meetings.

11. Supports the development and growth of telehealth services within the organization.

12. Provides consultation and education to internal and external stakeholders.

13. Utilizes approved video platforms to facilitate interactive, two-way communication with clients in accordance with policy.

14. Assesses clients’ technological, cognitive, and communication abilities for participation in telehealth services, including access to equipment and connectivity.

15. Prepares and orients clients to telehealth technology and provides troubleshooting support as needed.

16. Adapts clinical evaluation, intervention, and communication techniques to ensure effective, engaging, and distraction-free virtual care environments while demonstrating ongoing professional growth.

17. Maintain professional licensure, certifications, and stay current with clinical best practices.

  • Performs other duties as assigned.

Working Conditions

  • Primarily clinical and office-based, with occasional remote work.
  • Occasional travel to clinical sites or meetings may be required.
  • Access to confidential patient information; must adhere to privacy regulations.
  • Potential exposure to behavioral or crisis situations.
  • This position is non-union.
