Youth ACT Therapist

Join Nexus Family Healing as a Youth ACT Therapist in Mendota Heights, Minnesota. Nexus Family Healing is excited to add a hybrid-remote Therapist for our Youth ACT Program based out of Mendota Heights, MN! Youth ACT is an intensive, comprehensive and community-based rehabilitative mental health service.

Key Responsibilities

• Intake & Assessment Complete intake forms and diagnostic assessments.

• Ensure timely and accurate clinical documentation.

• Trauma Informed Care Complete CANS assessments using TCOM principles.

• Develop, update, and monitor individual treatment plans (ITPs) based on assessments and CAT/CANS.

Requirements

• Support discharge planning and post-care coordination.

• Case Management Coordinate referrals and collaborate with families, schools, and providers.

• Attend supervision, team meetings, and required trainings.

• Maintain compliance with all clinical and regulatory standards Community Collaboration Communicate client needs with care teams, families, and community partners.

Benefits & Compensation

• Salary: $55K - $68K

Work Schedule

• Full-Time position.

• Hybrid position - combination of remote and on-site.
