Key Responsibilities

This role involves delivering appropriate physical therapy evaluation and treatment to PACE participants across acute, restorative, or maintenance levels as required. Key duties include serving as a required member of the Interdisciplinary Team, performing comprehensive assessments, updating care plans, and training caregivers on mobility enhancement methods.

Requirements

Candidates must be a graduate of an APTA-accredited physical therapy program and possess excellent written and verbal communication skills for effective interaction with internal and external customers. Additional requirements include passing pre-employment screenings, holding a valid driver's license with compliant driving record, and demonstrating working knowledge of computers and Microsoft Office applications.
