Key Responsibilities

Provide in-home physical therapy visits including evaluations, reassessments, and fall risk assessments. Accurately document patient progress and communicate effectively with the care team.

Requirements

Candidates must hold a current Hawaii license as a Physical Therapist or Physical Therapist Assistant. A minimum of one year of clinical experience and a valid driver's license with reliable transportation are required.
